On my own behalf and on the behalf of my heirs and legal representatives and to the fullest extent permitted by law, I hereby release and discharge the Polk City Friends of the Parks and the City of Polk City and their respective board members, officers, and representatives, of and from any and all liability for injury, death, or damages and/or any other claims, demands, losses or damages, incurred by me in connection with any aspect of the 2k, 5k or 8k run and walk. I will not enter and participate unless I am medically able and properly trained, and by my registration/participation, I certify that I am medically able to perform this event, am in good health, and am properly trained. I agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation for any reason whatsoever. I attest that I agree to staying on the route of the race and abiding by race rules.
